Moving out of Home Support Pack

The "Moving Out of Home” sessions are designed to help neurodivergent individuals develop daily living skills of their choosing, in preparation for independent living. These sessions can cover home management tasks such as basic cleaning, meal prep and food shopping and can include an introduction to transportation, including familiarisation with public transport options. The aim is to provide foundational skills, resources and strategies as a starting point to build confidence for the moving out of home life transition.

Session 1: 
Set personal goals and identify priority skills.

  • Self-rate current performance and satisfaction using the modified COPM tool.

  • Choose key home management or transport skills to focus on.

Session 2: 
Finding the ‘Just Right Challenge’.
Learn an unfamiliar home management task.

  • Step-by-step teaching of the chosen task (e.g., cleaning or laundry) if needed.

  • Guidance to find the ‘just right challenge’ for success in the chosen task.

Session 3: 
Apply new skills and explore assistive technology if needed.

  • Practise the home management task.

  • Recommendations for assistive tools to support ongoing success in the task.

  • Option to create a home practice program for further skill-building.

Session 4: 
Practise again and address barriers.

  • Continue practising the same task in session.

  • Discuss and troubleshoot challenges with OT support.

Session 5: 
Reflect on progress and plan next steps for moving out of home.

  • Re-rate skills using the modified COPM.

  • Discuss resources and next steps for ongoing skill development.
